That's what my first thought too. But the word 'arginine' gave me a lot new perspective. So here's my theory, i think the boy is having a bad heart. He despise his currently sick condition as he's feeling restraint and eat only r-r-ringo (apple) lol for the sake of his health. And notice that there is stitches on his left chest where heart is laid. Also, this lantern fish that followed him everywhere is actually connecting to his heart. This can be seen clearly at . I think lantern fish is representing the breathing aid mechanism.
